As a former Iowa State University employee who was covered under our group life insurance and/or the voluntary term life insurances you are eligible to convert and/or port your coverage with the Principal Life Insurance Company.
Deadline dates for applying for coverage:
- Group Term (Basic) Life: You have 30 days from your coverage termination date (first of the month following last day worked) to apply and pay the initial conversion premium.
- Voluntary Term Life: You have 60 days from your coverage termination date (first of the month following last day worked) to apply and pay the initial portability premium for you and/or your dependent voluntary term life coverage.
- Dependent Term Life: You have 30 days from your coverage termination date (first of the month following last day worked) to apply and pay the initial conversion premium.
